He was predeceased by: his parents, Leroy Lewis and Melita Lewis (Boeckel); and his siblings, Thomas Lewis, Myrtle Golly and Irene Parker.
He is survived by: his wife Carol Lewis (Calhoon) of Sweet Home; and his children, Tony Lewis of Sweet Home, Rebecca Taylor of Tangent, Tiffany Troite of Sweet Home, Zachary Lewis of Sweet Home and Jeff Anderson of Canby. He is also survived by 20 grandchildren and 13 great grandchildren.
