He was predeceased by: his parents, William Mathis and Florence Mathis (Metcalf); and his son Jack Jr.. He is survived by: his wife Jodella; and his children, Rhonda Mathis, William Moises, Brian Moises and Bonnie Mathis. He is also survived by 1 Grandchild; 2 Great-Grandchildren and 4 siblings.
