He was predeceased by: his wife Shirley Jo Provance (Caligiore); his parents, George Luther Provance and Virginia Ella Pittman; his sister Linda Gayle Moore (Provance); and his grandson Austyn Lee McFadden.
He is survived by: his children, Andrea Marie McFadden (Doug), James Timothy Provance (Alecia) and Christopher Michael Provance; his grandchildren, Jaycee Marie, Kynzee JoLynn McFadden, Kelsey May, James Brennan, Madison Raelyn Provance, Shaelynn Autumn Provance and Cathryne; and his sister Donna Kay Forrest.
