He is survived by: his mother Marilyn York (Ayers); his wife Annette Cook; his children, Allison Galyon, Mike Cook, Michael Marchi and Anthony Cook; his niece Christina Kowitz (York); and his nephew Tyler York. He is also survived by loving Uncles, Aunts and cousins.
