She is survived by: her husband Mark Jacobs; her children, Logan Stewart (Bailey), Madison Collingham (Nathan) and Sophia Storm (Justin); her grandchildren, Ava Collingham, Reese Collingham, Harper Storm and Finley Storm; her parents, Janet Loerts and Wayne Blackburn; her stepmother Carolyn Blackburn; and her siblings, Corey Blackburn (Traci), Adam Loerts (Destiny), Megan Loerts (Shervin Nourparvar), Lincoln Loerts (Kristy Heide), Jessica Ortiz (Antonio Villalobos), Jeff Cave (Andrea) and Jon Worthey (Tina).
