He is survived by: his wife Colleen Ann Bushmaker (Legerski); his children, Tamara M. Bushmaker and Scott M. Bushmaker; and his siblings, Bill Bushmaker (Carroll) and Darlene Waddell (Harry). He is also survived by 4 grandchildren; and 2 great grandchildren.

Memorial contributions to Holy Redeemer Church and The Kids Hope Center through Bay Area Hospital in Jack's name will be much appreciated.
