He was predeceased by: his parents, Betty and Jack; and his brothers, Jack, Bobby and Ralph.
He is survived by: his wife Kayte; his sons, Jacob, Jason and Justin; his grandchildren, Gavin, Lillian, Jamin and Brandon; his sons, Joey and Jeremy; his sisters, Cindy, Deb, Judy, Pam, Rebecca and Sherry; and his caregiver Amber. He is also survived by nephews, nieces and cousins.
