He is survived by: his wife Sheri Porter Mack; his daughter Terri Mack of Sheri Sides; and his sister Jean Mack-Bettelyoun. He is also survived by 5 grandchildren, 4 great grandchildren.

During Jerry's battle with Alzheimer's, he continued to have rhythm and entertain everyone with his whistling and dancing.
