He is survived by: his children, Colton and Maria; his wife Darielle Church (Smith); his brother Duane; his daughter Kimberly Church-Quale of Florida; his grandchildren, Amber Wendtloch (Cody), Colton Stephenson, Trace Deciccio, Justice Deciccio, Declan Deciccio and Kaedan Deciccio; and his sisters-in-law, Kate Smith, Margi Wilson (Brad) and Anne Shirkey (Rick).
