He was predeceased by: his parents, William C. Cook and Mary B. Cook (Egbert); his siblings, Billy Cook, Herman Cook, Harold Cook, Duke Cook, Bob Cook, Mary K. James and Charles Carter; and his granddaughter Kandace Suzanne Richards.
He is survived by: his children, Bryan Cook (Kim) of Weatherford, OK, Doug Cook (Kim) of Weatherford, OK and Kay Richards (Mark) of Hobart, OK; and his grandchildren, Blake Cook of Weatherford, OK, Blair MakesCry (Derek) of Weatherford, OK, Chandler Cook (Samantha) of Atlanta, GA, Preston Cook (Brittany) of Beacon, NY, Andrae Cook of Norman, OK, Aaron Richards of Cresent, OK and Blaine Richards (Lindsey) of Hobart. He is also survived by 9 great grandchildren, as well as several cousins other relatives and friends.
Dr. Cook was always quick to give credit to his mother for instilling in him a strong faith-based foundation.
