He is survived by: his wife Nancy Slack; his grandchildren, Amanda Johnson (Ronnie), Justin Farris (Heather), Nicholas Farris, Cooper Slack, Haidyn Slack and Kendra Slack; and his great grandchildren, Andrew Johnson, Mason Farris, JJ Farris, Ali Farris and Audrei Hail.
