He was predeceased by: his parents, Joe David Cook and Ellen Orlovksy; his brother Joe David Cook Jr.; and his grandparents, Jim, Irene Cook, Dillard and Nora Holloway.
He is survived by: his significant other Sherrill Grippando; his children, Brad Cook (Tiffany) of Purcell and Lindsay Monnat (Danny) of Richardson, Texas; his grandchildren, Kaleb Cook of Purcell, Blake Cook of Purcell, Claire Monnat of Richardson, Texas, Mary Monnat of Richardson, Texas and Ember Lee Kennedy; his siblings, Geary Cook of Fort Worth, Wanda Santomenna of Albuquerque, Jana Scott (Curtis) of Wilburton and Julie Chronister of Wilburton; his uncle Don; his aunt Peggy Paige; and his cousin Mark Paige. He is also survived by numerous other family members and friends.
