He was predeceased by: his parents, Joseph Chuculate and Ira Belle Chuculate (Ballard); his grandparents, Wyley, Julia Chuculate (Carey), Dewitt and Eva Mae Ballard (Benge); his brothers, Wiley, Joseph, Kenneth and Richard Chuculate; his son-in-law Brandon O'Field; and his sister-in-law Linda Chuculate.

He is survived by: his pets, Miracle, Buster and Pussycat; his daughter Lisa O'Field of Tahlequah; his sister Julia Bighorse Ballard (Clyde) of Rose, OK.; his sisters-in-law, Gwen Chuculate of Coweta, OK, Judy Chuculate of Silverdale, KS and Patty Chuculate of Hulbert, OK.; his grandchildren, Mariah Osage (Taylor) of Tahlequah, Brandon O'Field of Tahlequah, OK., Brandilyn O'Field of Tahlequah, OK. and Obadiah O'Field of Tahlequah, OK.; his great grandchildren, Leyton Osage and Ayden Osage; his nephews, Boomer Chuculate, Kyle Bighorse, Anthony Bighorse, Andrew Lukens and Derek Chuculate; and his brother-in-law Clyde Ballard Jr.. He is also survived by 14 nieces and nephews, and 25 great nieces and nephews as well as friends and loved ones.
