He is survived by: his parents, Jack Tolbert of Amarillo, TX and Darlene Tolbert (Moore); his sons, Andy Tolbert (Sydney) of Davis, OK, Nick Tolbert of Sulphur and Daniel Tolbert of Calera, OK; his stepfather Fred Smith of Stratford, OK; his brothers, David Ray Tolbert and Joseph Elwood Jes Smith; his girlfriend Jaime Brown; and his girlfriend's children, Houston Kade Lewis, Alyvea Brown Pallbearers, Andy Tolbert, Nick Tolbert, Daniel Tolbert, Jes Smith, Ezekiel Miller and Duriel Flowers.
