He was predeceased by: his parents, James Floyd and Leo Belle Thomas Lewis; his wife Velta Lorraine Lewis; and his siblings, Daryl Lewis, Michael Forbach, Frankie Lewis and Cynthia McCosar.
He is survived by: his children, Casey Lewis, Gage Lewis and Staci Lewis; his siblings, Anthony Lewis, Phillip Lewis, Mitch Lewis, Jeff Lewis, Melissa Kilcrease (Lloyd) and Rolanda Casey; and his grandchildren, Jada, Tia and Dominique Tsotaddle.
