He is survived by: his wife Melissa Lewis; his children, Samuel Lewis, Jr., Isiah Lewis and Hope Lewis; his siblings, Jeff Lewis, Kenneth Lewis, Liz Lay and Jessica Lee; and his friend Alan Smith. He is also survived by numerous other family members and friends.
