He was predeceased by: his mother E. Lucile Crouch (Reich); his wife Madona M. Crouch; his father Leo Crouch (Peggy); his brother Robert L. La Force (Jan); his grandparents, David C., Willie F. Crouch (Clyatt), Robert and Mattie Reich (Reed); and his son-in-law H. Keith Parrish.
He is survived by: his daughters, Dana Lynn Parrish and Gina Beth Cherry (Bill) of Stigler, OK; his grandchildren, Amy Lynn Mooney (Jack), Noah David Smith (Christy) and Logan Ross Smith (Skylar) of Wynnewood, OK; and his great grandchildren, Chandler B. Smith, Cullen R. Stockton, Ferren R. Stockton, Laramie R. Stockton and Hannah Lynn Mooney.
In lieu of flowers, the family request that donations be made in David's memory to Church of Christ Disaster Relief Effort or Tipton Children's Home.
