He was predeceased by: his parents, Otis Lee "Jim" Fails and Geneva "Dina" Fails (Davis); and his brothers, James Fails, Bill Fails and Don Fails.
He is survived by: his wife Wanda Fails; his children, Randy Fails (Melissa Yarborough) of Georgetown, TX, Donna Wallace (Gregg) of Yukon, OK and Linda Langley (Roger) of Hugo, OK; his sister Sue Mann of Houston, TX; his grandchildren, Josh Fails, Jordan Fails, Andrew Fails, Savannah Emery, Lane Wallace, Jake Self, Jarrod Self and Cody Self; and his great grandchildren, Hadley Self, Emersyn Self, Samantha Self, Alexis Self, Kambree Self, Xadin Self and Baby Boy Self. He is also survived by many other relatives and friends.
