He is survived by: his sons, Murphy Collinsworth of Seneca, MO. and Fredrick Collinsworth of Broken Arrow, OK; his sisters, Donna Sue Brown and Kathy Woolverton; and his grandchildren, Ashley, Madison and Blayne. He is also survived by other relatives and friends.
