He is survived by: his wife Pamela Crutchfield (Smithson); his daughter Shawnya Slattery (Rod); his grandchildren, Riley Slattery of Starkville, MS, Walker Slattery of Richardson, TX and Keira Slattery of Richardson, TX; and his brother Johnnie Lee "Buddy" Crutchfield (Dean). He is also survived by several nieces, nephews, other relatives and friends.
