He was predeceased by: his mother LeFaye Hoffmann (Simmons); and his brother Timothy Jay Hoffman. He is survived by: his wife Leah Dawn Julien; his father Ronald Jay Hoffmann; his sister Angela Faye Calandra (Hoffmann); and his stepchildren, Takota Ray Hamilton and Devon Ray Lee. He is also survived by other relatives and friends.
