He is survived by: his children, Amber Hoffman of Midwest City, Laura Lavigne of New Jersey, Chad Hoffman (Yesenia) of Yukon and Bates Hoffman of New Jersey; and his siblings, Larry Hoffman (Nancy) of Muskogee, DeWane Hoffman (Louise) of McAlester and Janice Erb of Wellston. He is also survived by nieces, nephews, family and friends.
