She was predeceased by: her parents, Clarence Hawkins and Clara Hawkins (Surine). She is survived by: her mother-in-law Nelva; her husband Harold Choate; her children, Joshua Floyd (Jessi) of Chandler, OK, Gerald Floyd of Ponca City, OK, Tony Choate, Mark Choate (Tammi), Brian Choate, Matthew Choate (Tiara), Michael Choate (Shayna) and Tanna Floyd (Mark) of Blackwell, OK.; and her siblings, Everett Hawkins of Shidler, OK, Bill Hawkins (Shelly) of Pryor, OK and Janell Rhynard (Steven) of Braman, OK.. She is also survived by Numerous nieces and nephews, 30 grandchildren and 2 great grandchildren.
