He was predeceased by: his parents, Crawford Pettyjohn "Jack" and Rachel Ella Sullivan (Dunn); his brother Jerry Jack Sullivan; and his grandson Blake Andrew Newport. He is survived by: his caregivers, Haley, Carol and Kim; his wife Carolyn; his children, Terry, Michele Baskin (David) of Ponca City and Lesylee Hodge (Phillip) of Springdale, AR; his grandchildren, Tyler (Kacie), Ryan (Casey), Austin Sullivan and Shea Newport Base (Garrett); and his great grandchildren, Joey, Riley, Elijah and Conner.
