He was predeceased by: his father Lawrence Slovacek. He is survived by: his mother Joan Slovacek (Steichen); his stepfather Richard Welborn of Ponca City; his sister Mindy Benedict; his nephews, Blake and Ryan Benedict St (Haley); his stepsisters, Roxanne Waddel (Rick) and Drista Rogers (Tom); his uncles, Martin Steichen (Mildred) and Don Steichen (Shirley); and his aunt Lorene Artman (Wayne). He is also survived by cousins and relatives.
