He is survived by: his wife Vicky Ann McCullough; his children, BJ Cook of Paden, Sammy Cook (Kaye) of Paden, Karen Cook of Oklahoma City, Sinthia Lively (Jim) of Calvin and Keri Roberts of Oklahoma City; and his siblings, Fred Walley, Ron Johnson, Lee McCullough, Pete Walley, Nita Walley and Jeri Doss. He is also survived by nieces, nephews, family and friends.
