He was predeceased by: his parents, John Ballard and Thelma Ballard (Howell); and his brothers, Gary, Terry and Jimmy Ballard.
He is survived by: his wife Chris Ballard; his daughters, Sherry Lynn Swayze (Alex) of Eram and Lori Ann Jones (Brian) of Checotah; and his grandchildren, Matthew Swayze (Sarah), Aaron Swayze (Misti), Adam Swayze (Megan), Andrew Swayze, Amy Ford (Brandon), Alicia OHalloran (Charlie) and Bailie West (Robert). He is also survived by 16 great-grand children.
