He is survived by: his wife Gail of Midwest City; his children, Cary Hawkins of Edmond and Ashley Lefebvre of Choctaw; his mother Ann Wariner of Midwest City; his siblings, Marvin Hawkins of Choctaw and Teresa Roberts of Washington; and his grandchildren, Sean Lefebvre and Hadley Hawkins.
In lieu of flowers, the family is requesting that donations be made to the Stephenson Cancer Center in Dale's honor.
