She is survived by: her daughter Faith; her former husband Philip Smith; her daughter Hope Smith of Webb City, Missouri.; her husband Adam White; her mother Gale Lee (Doug Lee) of Miami; her stepfather Russell Ogle of Nevada; and her sister Jennifer Wormington of Monett, Missouri.. She is also survived by her cousins, aunts and uncles.
