He is survived by: his mother Ellen Kulick of Yukon, OK; his wife Kerrie Matos of Luther, OK; his sisters, Charlotte Denton of Yukon, OK, Susan Thomas of Warr Acres, OK and Angela Matos of Luther, OK; and his daughters, Amber Smith of Blanchard, OK and Amanda Simoneaux of Yukon, OK. He is also survived by four granddaughters; three grandsons; one great granddaughter; and numerous nieces and nephews.
