He was predeceased by: his parents, John S. Lewis and Bessie Lewis (Mixa); his wife Dorothy Pennington; his son David; and his siblings, John, Hank, Harvey, Arthur Lewis, Thelma Stewart, Elma Frazier and Betty Kriz.
He is survived by: his daughter Dr. Gail Lewis; his granddaughter Danielle Lewis; and his siblings, Russell Lewis (Margaret) and Margie Schlapbach.
