He was predeceased by: his parents, Lamar Cook and Verna Cook; his wife Mattie Cook; and his siblings, Alex, Eldern, Lilton and Mearline Butler.
He is survived by: his children, Jackie Cook Peoples, Dallis Cook (Yolanda), Renee Cook Boone (Jerry), Toni Swift Walker, Jessie and Dion Stewart; his grandchildren, April, Amanda, Dominique, Briana, Ayanna, Alex, Synphanie, Jasmine, Candrus and Dion Jr.; his great grandchildren, Jade, John, Jeremiah and Anthony; his siblings, Corrie Cook Certain and Dalton Cook; and his brother-in-law Drake Walker. He is also survived by nephews, nieces, friends and other relatives.
