He was predeceased by: his parents, Father Victor George Slavick and Florence Mabel Slavick; his significant other Mary Lynne Slavick; and his siblings, Bill Slavick, Jim Slavick and Vicki Taylor.
He is survived by: his children, Melissa Slavick Manes, Ken Slavick, John Slavick and Jason Slavick; and his grandchildren, Adam, Victor, Taylor, Ryan and Will. He is also survived by his great grandchildren and numerous other relatives and friends.
