He was predeceased by: his parents, Ray Ballard and Louise Ballard (McClary); his grandparents, Tommy, Grace McClary, Herman and Istus Ballard; and his brother-in-law Lane Whitehouse.
He is survived by: his children, Brynklee Ballard of Spiro, OK and Riggs Ballard; his siblings, Jean Ann Burris (Lawrence), Robin Whitehouse of Bristow, OK., Gary Ballard (Diane) and Mitchell Ballard (Lela) of Keota, OK.; and his wife Angelia Lewis. He is also survived by Numerous nieces, nephews and cousins.
