He was predeceased by: his parents, Bryce Cook and Naomi Cook; and his brother Lauren Leroy Cook. He is survived by: his sisters, Merrietta Miller of Hydro, OK and Lynette Cook; his niece Janelle Zabel (Randy Zabel); and his great-nephews, Ryan Zabel (Lauren Aprile), Colby Zabel and Shane Zabel.

In Dallas, Dale worked for Goodwill sorting donations and general custodial details.
