She was predeceased by: her parents, Melvin Stanley Willard and Ola Lee Willard (Linder); and her siblings, Donna Mooney, Joy Walkup, Laquitta Kaiser and Benny Wayne Willard.
She is survived by: her husband Vernon Cook; her daughter Misty; her son-in-law Joe Clower of Holdenville; her grandchildren, Lewis Black Blankenship of Ada, Brittany Blevins (Kolby) of Ada and Mayson Carpitcher of Holdenville; her great grandchild Katie Rae Blevins of Ada; and her sisters, Ola Mirrell of California and Carolyn West of California. She is also survived by nieces and nephews, other family members, and many friends.
