He was predeceased by: his son Levi; and his parents, Harvey Cook and Ellen Long. He is survived by: his daughter Angie; his wife Kathy; his children, Jonna Cook (Angie Cook) of Wichita and Marcus Cook of Oklahoma City; and his sisters, Deborah Hansard of Lubbock and Marsha Edwards of Hobart.
