He is survived by: his mother Carolyn Booze of Hinton, OK; his siblings, Cathy Whitaker (Bob) of Tulsa, OK, Jon Booze (Sharon) of Hinton, OK, Deniece Ditzler (Eddy) of Edmond, OK and Michael Booze of Hinton, OK.; his nieces and nephews, Shanda, Linda, Greg, Jerid and Shaina; and his great-nieces, Ashton, Addison, Chace, Camron, Caitlyn and Madelyn.

In lieu of flowers please make donations in Creig's memory to the American Cancer Society.
