He was predeceased by: his parents, Billy Cook and Clara Cook. He is survived by: his wife Donna Cook; his children, Jessica Glass (Edward) of Glenpool, OK and Daniel Cook of Glenpool, OK; his grandsons, Jared (Katie), Jackson (Kennedy) and Trevor of Sapulpa, OK; his sisters, Vickie Cash of Hampton, GA and Debbie Crider of Dallas, TX; and his niece Tracy Cash of Jonesboro, GA. He is also survived by several nieces and nephews.
