She was predeceased by: her parents, Lloyd Alfred and Dorothy Marguerite Smith (Hazlett); her grandmother Willie Bell Colyer; and her husbands, Choppy Howell and James Ray Polk.

She is survived by: her daughter Carla; her son-in-law Randy Loveday of Frederick, Oklahoma; her son Jeff Howell of Frederick, Oklahoma; her granddaughters, Lindsey Loveday of Edmond, Delanie Howell of Enid, Deighton Howell of Edmond and Dannyn Howell of Wynnewood, Oklahoma; her great grandchild Evelyn "Evie" Howell of Enid, Oklahoma; her half-sister Robe Mourer of Tipton, Oklahoma; and her sister-in-law Peggy Burrow of Denison, Texas. She is also survived by nieces, and nephews.

Memorial contributions may be made to the First United Methodist Church in memory of Jean Howell Polk.
