He was predeceased by: his parents, Henry Ballard and Ollie Ballard (Hunter); his siblings, John Henry "Jiggs" Ballard, Lawrence Ballard, Howard Ballard, Marvin Ballard and Laverne Ballard Knight; his grandmother Mae Ballard Huggard Gustavson; and his son Stephen Ballard.

He is survived by: his wife Yoko Ballard; his children, Mark Ballard of Oklahoma City, Robert Ballard of Horse Cave and Kayleen Smith of Sand Springs; and his siblings, Rowland Ballard of Granbury, Wanda Ballard Moore of Huntsville and Jane Ballard Compton of Picayune. He is also survived by grandchildren, great grandchildren, nieces, nephews, friends.
