He is survived by: his wife Sally Ann Cook (Hume) of Enid; his children, John David Blancuzzi (Francis) of Ponca City, Mary Elizabeth "Betsy" Bell (Stacie) of Pilot Grove, MO, Kathy Naheed Grisham (Jason) of Angel Fire, NM, Jessica Lynn Cook (Nate) of Enid and Justin Hume Cook (Jessica) of Mount Airy, MD; his grandchildren, Nicole, Niko, Garrett, Sydney, Haylee, Danielle, Tristin, McKenzie, Stem, Andrew, Kiersten, Matthew, Bodi, Ryder and Kinley; and his siblings, Jerry Cook (Charlene) and Sharon Stewart (JD). He is also survived by his family, friends.
