He was predeceased by: his parents, Perry Thornton Kuykendall, Sr. and Betty Jean Hinkle Kuykendall; his siblings, Pete, Robert and Kathy Nelson; his brother-in-law Rex Ridenour; and his niece Jennifer Nelson.
He is survived by: his wife Phyllis Ridenour; his children, Chris, Kyle Kuykendall and Stephanie Johnson (Brad); his sister-in-law Annelia Ridenour; his grandsons, Caleb and Tyler Johnson; and his nephews, Kevin Ridenour (Ashley), Matt Ridenour, Jonathan Nelson and Jimmy Kuykendall (Mary Anne).
The family suggests memorials to the Clinic on the Hill c/o Quail Spring Baptist Church, 14613 N. May Avenue, Oklahoma City, Oklahoma 73134 either with Woody Kuykendall or The Clinic on the Hill in memo please.
