He is survived by: his parents, Herman Ray Cook Sr. and Maggie Evelyn Herrold; his wife Karen; his children, Lisa Rondina (Gary) and Kevin Cook (Tammy); his grandchildren, Tiffany Grant-Evans (Brett), Tara Grant, Kory Cook (Lakan), Taylor Grant, Ryan Rondina and Cody Smith; his great grandchildren, Rowan, Sloane Cook and Olivia Rondina; his brother Ron Cook (Paulette); and his sisters-in-law, Coleen Cook and Carlee Cook. He is also survived by numerous nieces and nephews.
