He was predeceased by: his father James L. Hazen; his stepfather Woody Wright; and his mother Sally Wright. He is survived by: his son John Hazen of Tulsa; his grandchildren, Ryder, Kylie, Aiden, Braden, Cayden and Zaden; and his sisters, Jill Hazen of Pryor and Susie Taylor (Curtis Taylor) of Pryor.
