He was predeceased by: his parents, Johnny Lynn Forbus and Wanda Marie Forbus Caldwell. He is survived by: his children, Cortni Hawkins (Scott) of Moore, Sydney Dommer (Dave) of Raleigh, NC, Hannah Forbus and Harlee Walker of Ada Oklahoma; his sisters, Dina Newman (Tod) of Subiaco Arkansas and Debbie Chandler (Steve) of Moore Oklahoma; and his grandchildren, Sawyer, Camdyn, Sloane Hawkins and Emmett Dommer.
