She is survived by: her former husband Blake Mayhew; her children, Dylan and Gracie; her parents, Ron Cody and Vickie Cody; her siblings, Heath, David, Robert, Angela, Melony, Joe, Zackary, Devon and Chelsey; her grandmother Shirley Houtz; her uncles, Dick Forbis, Dave and Mike; and her aunt Anita. She is also survived by many nieces, nephews, and cousins.
