He was predeceased by: his brothers, Gary Blake Cook and Daren Ray Cook; and his grandparents, Lee, Ella Cook, Henry and Maggie Cochran.
He is survived by: his parents, James Cook and Juanita Cook (Cochran); his wife Janet (Avant); his children, Lisa Van Schuyver (Drue), Carla Nicholas (John) and Eric Smith (Rebecca); his grandchildren, Kiara, Diamon, Levi, Austin, Lane, Livia, Jackson, Logan, Devin and Chelsea; and his siblings, Tim Cook (Jackie) and Susan Bogle (Allan). He is also survived by numerous nieces and nephews.
