He was predeceased by: his grandparents, Charles Arvid, Ruth Ball Levy (Broadrick), Leonard, Frances Foster, Dexter Levy, David Levy, Murry Foster and Sue Foster; and his cousin Anna Siler.
He is survived by: his parents, Dana Levy and Mary Levy (Foster); his children, Matthew Levy and Allison Levy; the mother of his children Sherry Levy; his brothers, Jim Levy and Jeff Levy (Becky); and his cousin Matt Foster. He is also survived by many other nieces, nephews, extended family, and friends.
