He was predeceased by: his parents, Dr. Jack C. Mileham Sr. and Mary Moran (Skinner). He is survived by: his sister Marilyn Schweizer; and his stepsister Rhonda Bennett (Mileham). He is also survived by two nephews and one niece.
In lieu of flowers, the family has asked that donations be made to the Chandler Youth Baseball Association or the Chandler High School Baseball Program in Jack's honor.
